2009 Request for Nominations

Please nominate candidates for the 2009 Public Health Heroes.
The heroes will be honored at an awards ceremony in Spring 2009.

This unique honor was initiated in 1996 by the School of Public Health's Policy Advisory Council to "broaden people's awareness and understanding of the public health field by recognizing individuals and organizations for their significant contributions and exceptional commitment to promoting and protecting the health of the human population."

The Policy Advisory Council typically selects four Heroes, including three individuals—representing an international, national and regional honoree—and one organization.

First Round Deadline: May 30, 2008.

Please submit the online nomination form, which includes space for a one-page letter.

In preparing your nomination, please cite: accomplishments, exemplary service, academic background or association, and criteria that characterize the individual or organization as a Public Health Hero.

Criteria: A nominee may have had a significant influence on an important legislative health policy issue; prevented the spread of a disease or contributing conditions; been a public policy advocate; aided an at-risk or vulnerable population faced with serious threats to their health; acted to safeguard or improve the environment; served with distinction as a mentor or teacher; or made a significant biotechnological, drug or diagnostic discovery.
 
Second Round Deadline: July 11, 2008
 
Approximately 12 nominations will progress to the second round of the nomination process. Nominators will be requested to submit a more extensive nomination letter and at least one letter of support.

Eligibility: Only individuals and organizations external to the School will be selected as Public Health Heroes. Current staff, students, faculty, emeriti, and affiliated organizations, such as centers, are not eligible for nomination.

Note: Public Health Heroes must be available and willing to accept the award in person at the awards ceremony in Spring 2009.
